How to Get Started: Forms & Guidelines
In order to participate in athletics at River Bend Middle School, students must complete the following:
1. Students must have a current physical, dated within the current year, on file at the school in order to participate in open gym or tryouts.
2. Make sure all forms are complete.
3. Turn in completed paperwork to the coach BEFORE tryouts.
4. Communicate with the coach and check both the school calendar and our Facebook page for practice/tryout/game times.

Please print and complete the following forms prior to your student's participation in any athletic activities, including workouts and tryouts. Printed packets are also available for pickup in the school office.

All students must be covered under insurance in order to participate in school sports. If you do not have insurance for your child, below are links to affordable insurance for your student athlete.
